What is our Center for Advanced Diabetes Care?
This program encompasses a physician panel which includes a nephrologist, a diabetologist, an optometrist, a podiatrist, a psychiatrist, a diabetic certified Nurse Practitioner, and a vascular surgeon in order to monitor and treat the co-conditions often present with diabetes. Interdisciplinary patient care involves nursing, dietary, therapy, and activities working together to coordinate each client’s specialized plan of care. We monitor our patient outcomes and have proven reductions in Hba1c, hospitalizations, skin breakdown, and infections. Follow-up care includes an on-site outpatient clinic which includes outpatient therapy as needed.
Why is Diabetes Care so important to us?
Diabetes is the leading cause of blindness, a major cause of premature death and disability, and is the primary cause of 70% of lower body amputations, just to mention a few statistics. With an 80% increase in the number of diabetics in the last 10 years and 2,200 people diagnosed with diabetes each day, we feel that effective management of this disease is essential. Our Diabetes Program provides interdisciplinary patient care including our facility endocrinologist, nursing, dietary, therapy, and activities working together to coordinate each client’s specialized plan of care. We monitor our patient outcomes and have proven reductions in Hba1c, hospitalizations, skin breakdown, and infections.
What is Respiratory Care?
This specialized health care model is designed to identify, treat, educate and manage acute and chronic diseases related to breathing and/or the absorption and distribution of oxygen to the body. Our exceptional and caring team provides compassionate care for recently hospitalized patients and long-term care residents with around the clock nursing attention. Our goal is to become the facility of choice for post-acute respiratory disease management.
Why do we provide Pulmonary Disease Management?
This onsite respiratory therapy program improves the pulmonary health and stability of our residents in order to improve their quality of life and reduce unnecessary returns to acute care hospitals. Our trained pulmonary team treats individuals with conditions such as pneumonia, COPD, asthma, bronchitis, emphysema, Black Lung Disease, and pulmonary hypertension.
What makes our Behavioral Health Services unique?
For our cognitively impaired residents, we feature customized dementia and behavioral care in our cutting edge Serenity HealthCARE™ program. The program provides clinically appropriate medical care by highly trained staff in specially designed environments featuring clutter free halls, open access to inside and outside areas, rest stops, male and female day rooms, and apartment style resident rooms. All staff receives advanced training on how to effectively provide care for this unique population.
